Step I - Online Medical History Form
All athletes must read the Introduction Letter outlining the new online medical history form.
New Athletes: To enter your information visit www.swol123.net. The first time you visit the website you will need to enter your Wesleyan email address and click Get Password. All students should use their Wesleyan email address. Check your email for your password and log on to www.swol123.net. Students should complete ALL tabs on the "my info" tab and all questions on the "medical history" tab (click "yes" or "no" and enter details if "yes"). Please disregard the forms and print tabs.
Returning Athletes: To enter your information visit www.swol123.net. Returning students should sign on with their existing password or reset their password. Students should verify/update demographics on ALL tabs on the "My Info" tab and then answer the yes/no questions on the "Medical History" tab. Please disregard the forms and print tabs.
II - Medical Clearance Packet
Please complete all of the information on the Medical Clearance Packet form. Please complete all sections of the form including medical records, sickle cell, ADD/ADHD, pre-participating screening acknowledgement, insurance information, Athletic Injury Care privacy practice, assumption of risk and informed consent and injury, illness and concussion reporting acknowledgement form. After filling out the entire packet please print the document and sign where indicated. Please bring the completed form to your medical screening.
Step III - Report to Medical Screening
All athletes are required to go through medical clearance screening prior to participating in official team practices. All Spring sport returning athletes and those frosh that missed their fall medical clearance should report to the Athletic Injury Care office at the assigned date and time, as scheduled below:
Important Information About Your Medical Screening
1. Do not work out in the 3 hours prior to your medical clearance time.
2. Please upload a photo of the front and back of your medical insurance card or bring a photocopy with you to the screening.
3. If you are unable to attend at your scheduled time please email Joe Fountain at email@example.com.
Step IV - ImPact Testing Schedule
First Year and Transfer Athletes in the following sports are required to complete baseline ImPact testing:
All baseball, softball, men's lacrosse and women's lacrosse athletes must have an ImPact baseline on file. If an athlete missed their baseline from the fall of 2013, they must make it up.
