Camp Physical Form (All campers must have this) These forms maybe mailed one week prior to camp or brought to the first day of camp: Mary Bolich (phone 860-685-2929) Freeman Athletic Center 161 Cross St. Middletown, CT 06459 All campers must have all forms (including physical), and be checked in before they will be allowed to participate in camp. All medications can only be self-administered!! All medications must be brought to camp in the original prescription bottle with the original label with child's name on it. Any medications brought to camp in either a different bottle or with no prescription label will not be accepted. Any questions please call.
