The online form is a 3-page PDF document. To complete the form online and submit it electronically, you must have Adobe Reader plugin installed on your system. Adobe Reader plugin works directly in the browser window, allowing you to type in the form fields.
At the end of the document is a SUBMIT button--click this when you have completely filled in the form. It will be mailed to the Chemistry Department. You should receive a confirmation message upon submitting it. You are required to supply an email address in the form. This provides the Chemistry Department with a means of getting in touch with you easily.
