How to use this form
Use this form if you need to add an available course to your current registration.  You may also add a course through GLSP Online Registration  in your e-portfolio.  If you are withdrawing from one course and adding another, please submit both this form and a withdrawal form.  Please submit this completed form by fax, mail, or deliver in person to the GLSP office before the class begins meeting.  Students may also add courses to their registration through the online registration section of the GLSP student e-portfolio (online registration closes 5 PM Sunday before the first week of the term). Once a course begins meeting, registrations are not accepted for it. 

Adding and Dropping Courses
If, during the first week of the term, a student wishes to withdraw from one course and register for another course that has not yet met, the GLSP will accommodate this subject to course availability, provided that the latter course carries the same number of credits.

Switching from Audit to Credit
Changing a course’s registration status from credit to audit counts as a withdrawal from the course with any attending tuition liability, and new registration as an auditor of the course, with any applicable late fees.  Students who wish to change registration status from audit to credit or credit to audit must do so before the first class meeting.

Registration for any course constitutes an obligation to pay tuition for that course and to complete the academic requirements for that course.  To obtain relief from academic penalty and tuition liability, a student must submit a completed, dated, and signed copy of the official withdrawal form to the GLSP office within the specified deadlines.  A student is not relieved of the obligation to pay tuition or to complete the academic requirements of a course simply by informing the instructor of a decision to withdraw or simply by not attending class.
