State Immunization Laws
Connecticut State law requires that students registering at post-secondary schools demonstrate that they meet criteria of adequate immunization against measles and
rubella. Persons born before January 1, 1957 are exempt, as they are presumed to
be naturally immune. To demonstrate compliance with the state's immunization criteria
students must submit to Wesleyan Student Health Services the following two
items:
(1) the GLSP
cover sheet, and
(2) one of the following forms of proof of immunity.
Proof of immunity may consist of the following:
- A certified laboratory report showing that
the student has had protection against measles and rubella; or
- A signed letter from the student's physician stating that the student
has had two doses of measles vaccine at least one month apart—the first dose
received after 1968, the second dose received on or after January 1,
1980—and has had one dose of rubella vaccine. The dates the doses were
received must also be provided; or
- A signed letter from the student's physician stating that in the
physician's opinion such immunization is
medically contraindicated and the specific reason(s); or
- A signed letter from the student's physician, or from the director of health in the student's present or
previous town of residence, stating that the student has had a confirmed
case of such disease; or
- A signed affidavit from
the student stating that such immunization would be contrary to his or her
religious beliefs; or
- A photocopy of the student's high school diploma or transcript showing
that the student graduated from a public or nonpublic high school in the
state of Connecticut in 1999 or later.
These documents may be mailed or hand delivered to Wesleyan Student Health
Services at 327 High Street, Middletown, CT 06459 or faxed to 860-685-2471.
We recognize that students
may learn of the GLSP too late to ensure that all of these forms are on file at
the GLSP office prior to the first day of class. Students may enroll and
begin their first course in the GLSP prior to the delivery of these documents to
Wesleyan Student Health Services. This grace period will not extend to a
second course. If any of these documents have not been received at the
completion of the student's first course, the student will not be eligible to
register for a second course until the documents have been received.
Wesleyan Student Health Services does not provide services to GLSP students
beyond processing immunization compliance documentation. If you need
to have the MMR vaccination administered, you should speak with your regular
physician.

Requirements (pdf) and on the
