TRAVEL CONSULTATION

The Health Center offers travel consultation services for students traveling abroad.  Please schedule your travel consultation at least two (2) months before your departure to allow sufficient time for immunization series.

Please download the travel consultation form prior to your appointment.  Complete page one of the form before your appointment and check with your health insurance carrier re:  coverage for travel-related expenses (e.g. vaccines and anti-malaria medication).  You may be asked to reschedule your travel consultation if you fail to bring your completed form with you to your appointment.  Please note your travel appointment may last a full hour.
