Plan Eligibility

 New hires are eligible for coverage without having to provide proof of good health if they apply within 30 days of the date of hire. All actively at work employees whose weekly scheduled hours are 17.5 or more are eligible to apply for the plan but will be medically underwritten.  In addition, the following are also eligible to apply with medical evidence: spouses or partners, parents, parents-in-law, siblings and their spouses, adult children and their spouses, grandparents, grandparents-in-law and retirees.

 

Plan Highlights

What services are covered?

Nursing home care, alternate facility care, community based professional care and home health care.

What are the benefits?

There is a 90 day elimination period with a choice of daily benefit amounts: $200, $250, and $300 payable for 5 years.

What are the premiums?

This is a voluntary plan paid in full by the employee.  Premiums can be deducted from your paycheck for yourself and your spouse/partner.  All others are direct-billed.
