ACADEMIC COURSE RESERVE REQUEST FORMS
& Course Reserve Information for Faculty

Please use one of the following Reserve Request formats to provide a complete citation of items being requested for course reserve.

When submitting photocopies, or pdf's, please include a copy of the title page and the verso (flip page of title page which includes all the copyright information.)

BRIEF INFO:

OLIN:
Olin Reserve processes both electronic and/or physical items for over 100 courses each semester. The reserve staff relies on a Reserve request containing a complete citation for each unique title that will be used for academic course work. Please use one of the above form types to submit a reserve request.
MUSIC:
Requests for musical recordings should be directed to Music Library staff. For online access to recordings, please direct all requests to the Music Reserve Staff.
