Course Reserve Request Forms
Please use one of the following forms to provide submit requests for electronic, print, or media reserve materials.
Step 1. What type of of item are you putting on reserve?
Personal Copy - an item I own and will provide to the library as a physical copy (e.g. book, printed article, printed manuscript/draft, CD, DVD, etc.)
Audio recording (e.g. music, spoken word)
Another way: For items owned by a CTW library, you may also submit reserve requests by finding the item in the Library Catalog, then choosing "Make a Request"
