Employment Questionnaire

You must complete the following form and attend a mandatory information session before beginning any work for the Office of Community Service and Volunteerism.  We strongly encourage you attend the infor session before filling out this form.  You will receive an e-mail from the OCS once we verify your information and confirm your eligibility. 

For literacy positions, please contact the coordinator(s) of each program for placement information -- OCS approves your eligibility to work and you get hired at the program level.

For community-based and van driver positions, please fill out the form and the OCS Director will follow up with you directly.
