Employment Questionnaire

You must complete the following form and view our Employment webinar before beginning any work for the Office of Community Service and Volunteerism. We strongly encourage you to view the webinar before filling out this form. You will receive an e-mail from the OCS once we verify your information and confirm your eligibility.

For literacy positions, please contact the coordinator(s) of each program for placement information -- OCS approves your eligibility to work and you get hired at the program level.

For community-based and van driver positions, please fill out the form and then e-mail Cathy Lechowicz (clechowicz@wesleyan.edu).

Non-work study students are eligible to apply, you will not get paid for the program and you do not need to complete this questionnaire.
