COACHING DOCUMENTATION
  Unit Log and Reflection - one for fall and one for spring
  Planning & Reflection Skills Self-Assessment - one for each session (12 in fall, 12 in spring)
  Classroom Session Notes & Data - one for each session (12 in fall, 12 in spring)

TERC FORMS REQUIRED FOR TEACHERS AND PARENTS
  Permission Form for Teachers who are not being coached - one each teacher who is part of the TERC study
  Permission Form for Teachers who are being coached - one for each "coachee"
  Information Sheet for Parents and Students of all Teachers who are part of the TERC Study- one for each student

LEARNING COMMUNITY DOCUMENTATION
  Learning Community Meeting Reflection - one for fall and one for spring

PROFESSIONAL PRESENTATION
Make a peer-presentation once during the fall and spring at a team, department, or school meeting about a topic of your choosing, such as a presentation about the LAMSS Project, specific topics learned during your LAMSS training, Edusoft, etc.
  Professional Presentation Reflection - one for fall and two for spring

TERC JOURNAL
The outside evaluator, TERC, will contact you directly for this information
