Congratulations, Pre-College students, we are looking forward to meeting you!

Please read all of the documents below and print out then sign the forms. Completed forms should be faxed to 860-685-2901 or scanned and emailed to precollege@wesleyan.edu by June 26, 2013.  If you have any questions, feel free to contact us at 860-685-2005 or precollege@wesleyan.edu.

1.  Student Handbook  (Admitted students and parents/guardians must read this before signing the forms below)

2.  Medical Release Form  (Signed by parent/guardian)

3.  State of Connecticut Health Form  (Signed by parent/guardian and student's physician)

4.  Disciplinary Code Acknowledgement Form  (Signed by parent/guardian and student)

5.  Weekend Away Form (Signed by parent/guardian if applicable)
