The Registrar's Office will be
closed on Friday, July 4th. Please plan accordingly when ordering
transcripts. |

TRANSCRIPTS: Transcripts may only
be released with the written consent of the student, per the Family Education Rights and Privacy Act of
1974. If you wish to order transcripts,
we will need a SIGNED, WRITTEN
request from the student. Please use the transcript request form available
below. Because we are required to have a signed release, we cannot process
telephone of e-mail requests.

Current Students:
Stop
by the Registrar's Office and fill out a Transcript Request Form or download the
Transcript
Request Form using
Adobe
Acrobat Reader and bring it to the Registrar's
Office.

Alumni/Inactive Students:
Download the
Transcript
Request Form using
Adobe
Acrobat Reader
and send it to the Registrar's Office. Please also include the following:

a check or money
order made payable to Wesleyan University (see fee schedule below)

if sending
forms, also include specific directions for completion
TRANSCRIPT FEE: The fee is $5.00 (U.S.
currency only) per copy for
alumni and non-active students (payable by check or money order) and $3.00
(U.S. currency only) per
copy for active students (payable by cash, check or student account). Full
payment must be received with your transcript request unless you are a current
student and are billing to your student account. Make your check or money order payable to Wesleyan
University and mail it with your signed written request to:

ALL OFFICIAL TRANSCRIPTS ARE SENT IN SEALED
ENVELOPES

Normal processing time of transcripts is FIVE days from the date received.
-
We do not send unofficial copies of
transcripts to students or alumni.
-
No transcript will be released to or
for any student with an outstanding financial obligation.
-
The transcript does not include
credit/unsatisfactory written evaluations. If requested on the form under
additional comments or instructions, all copies of written evaluations on
file with the Registrar will be included with the transcript.

We are unable to accept credit card payment.
REPLACEMENT
DIPLOMAS:
Wesleyan will provide a replacement diploma in
situations where the original diploma was lost or damaged or if there has been
a legal change of name. Replacement diplomas reflect the current diploma
format and display the current Wesleyan president's signature.
Replacement diplomas also bear the words "Replacement Diploma" on the bottom
of the diploma. The original diploma must be returned when requesting a
replacement due to damage or a name change. In order to protect the
integrity of the diploma process, we require the following when requesting a
replacement diploma:

Application for Replacement Diploma. Your signature must be
notarized on this form.

Photocopies of two (2) forms of
identification from the following: passport, birth certificate, driver's
license, social security card.

If your name has been legally changed, you
must enclose a copy of the legal name change document.
Please also enclose a check made payable to
Wesleyan University in the amount of $50.00. Send the
application, accompanying forms of identification, original diploma, and check
to:
