- Speaker/Music Request Form
(PDF)
If you are bringing a speaker, musician, or other performer to campus, you must complete this form in addition to the Allocation Request Form.

This form outlines the conditions to which student groups are held regarding their involvement with the SBC. All groups must complete this form at the
beginning of each school year.

All student publications must fill out this form in addition to the Allocation Request Form.
WSA Office Forms
- Financial Request Form for Paying and Reimbursing Wesleyan Students and Staff (PDF)
This form is used if you are requesting reimbursement or payment for someone enrolled in or employed by Wesleyan University. Before submitting this form, please ensure that the requested amount of money is available in your 866 or 877 account. You need to have an original copy of a contract or receipt attached to this form in order to be reimbursed or paid. To be reimbursed for travel, you must attach a printed copy of the Mapquest directions (including total mileage). Please complete the form in its entirety. Any blank areas will delay the reimbursement.

This form is used if you are requesting reimbursement or payment for any person
NOT enrolled in or employed by Wesleyan University. Before submitting this form, please ensure that the requested amount of money is available in your 866 or 877 account. Department Contribution Form (HTML)
This electronic form is to be used when student group leaders request funds from academic or administrative departments to support an event and/or activity. You can either direct them here to the WSA website or provide the following link: http://www.wesleyan.edu/wsa/contributionform.html
It is important that you provide the department with the following information when you make your request so that they can complete the form:
- Student Group Name:
- Account Number:
- Event Name:
- Event Start Date:
- Event End Date:
- Student Name:
- Student Email Address:
- Student Phone:
The expenses incurred by your event will normally be paid directly from the department's account and will not be transferred into your group's Income Account. The WSA Office Coordinator will handle all financial transactions.
- Athletic Official/Coach Receipt Form for Club Sports Teams (PDF)
This is a contract to be used by club sports teams when hiring an athletic official such as a referee or coach. In order to pay the official, please complete this form and return it to Lisa Hendrix in the WSA Office.
