SBC Forms

  • Speaker/Music Request Form (PDF)
    If you are bringing a speaker, musician, or other performer to campus, you must complete this form in addition to the Allocation Request Form.
  • SBC Contract
    This form outlines the conditions to which student groups are held regarding their involvement with the SBC. All groups must complete this form at the beginning of each school year.
  • Publications Questionnaire (PDF)
    All student publications must fill out this form in addition to the Allocation Request Form.

WSA Office Forms

  • Financial Request Form for Paying and Reimbursing Wesleyan Students and Staff (PDF)
    This form is used if you are requesting reimbursement or payment for someone enrolled in or employed by Wesleyan University. Before submitting this form, please ensure that the requested amount of money is available in your 866 or 877 account. You need to have an original copy of a contract or receipt attached to this form in order to be reimbursed or paid. To be reimbursed for travel, you must attach a printed copy of the Mapquest directions (including total mileage). Please complete the form in its entirety. Any blank areas will delay the reimbursement.
  • Financial Request Form for Paying and Reimbursing Non-Wesleyan People (PDF)
    This form is used if you are requesting reimbursement or payment for any person NOT enrolled in or employed by Wesleyan University. Before submitting this form, please ensure that the requested amount of money is available in your 866 or 877 account. You need to have an original copy of a contract or receipt attached to this form in order to be reimbursed or paid. To be reimbursed for travel, you must attach a printed copy of the Mapquest directions (including total mileage). Please complete the form in its entirety. Any blank areas will delay the reimbursement.
  • Check Request Dates (PDF)
    This form is used to provide a list of submittal dates for students requesting reimbursement for expenditures. If you need to request a check for reimbursement, you should complete a financial request form and attach an original receipt/invoice and submit all documents to the WSA Administrative Office. The due dates to submit a request for reimbursement are listed in this form. You can pick up the date listed on the right after 12pm.
  • Deposit Forms for Student Group Accounts (PDF)
    After filling out this form, all groups must count their money with an office assistant or the WSA Office Coordinator. This will take approximately 15 to 30 minutes or longer depending on the deposit amount. All change must be wrapped beforehand and the student group name and income account number must be written on each wrapper. You can pick up coin wrappers from the WSA Administrative office in Usdan. We can not accept deposits after 4pm.
  • Department Contribution Form (HTML)
    This electronic form is to be used when student group leaders request funds from academic or administrative departments to support an event and/or activity. You can either direct them here to the WSA website or provide the following link: http://www.wesleyan.edu/wsa/contributionform.html
    It is important that you provide the department with the following information when you make your request so that they can complete the form:

  • Athletic Official/Coach Receipt Form for Club Sports Teams (PDF)
    This is a contract to be used by club sports teams when hiring an athletic official such as a referee or coach. In order to pay the official, please complete this form and return it to Lisa Hendrix in the WSA Office.
