Athletic Official/Coach Receipt Form for Club Sports Teams

This contract is to be used by club-sport teams when hiring an athletic official, referee or coach, who is NOT enrolled or employed by Wesleyan university. If this is not the case, please use the Wesleyan Employee Timesheet.

Cardinal Print and Copy Form

Wesleyan has an on-campus printing services department, Cardinal Print and Copy located in Usdan 138. Student group leaders should use this form, if they want to take advantage of this service.

Financial Request Form – Non-Employees and Outside Contractors
This form is used if you are requesting reimbursement or payment for any person or outside contract NOT enrolled in or employed by Wesleyan University.

Financial Request Form – Wesleyan Students and Employees

This form is used if you are requesting reimbursement or payment for someone enrolled in or employed by Wesleyan University.

Student Group Activity Waiver Form
This form is used if you sponsoring an off campus event and need a waiver form.

Timesheet - Wesleyan Employees

This form is used to pay Wesleyan faculty, staff or temporary worker.

Timesheet - Wesleyan Students
This form is used Wesleyan undergraduates or grad students.

W-8BEN Form
Form W-8 is filled out by foreign entities (citizens and corporations). There are many different types of W-8 forms such as: W-8BEN, W-8ECI, W-EXP, etc. We are providing the W-8BEN Tax Form on our site. However, please be advised that you should contact the finance office to inquire about which form to use.

W-9 Form for BUSINESS
W-9 Form for Individuals
If your student organization hires an individual or corporation for contract work they are required to complete a W-9 Form, titled Request for Taxpayer Identification Number and Certification. There are two forms; one for business and the other for individuals. Please choose appropriately.

If your group has been officially recognized and you want to request office space at 190 High Street, please use this form.

If you need to transfer funds between groups, or between account codes within one group smartkey, you must complete this form.
